Beginning Fall of 2017, any student who wishes to participate in a sport must have a sports physical BEFORE the first practice.

There will be no exceptions!

The physical must be done within 13 months of that sport. So, a physical done in June 2017 will cover a student for all sports during the school year.

REMINDER: The only date that will be accepted is the date of the exam noted on the top of page 2 of the form, not the date next to the physician's signature!

If your child has a mandated or yearly physical, make sure your physician checks off the sports participation section.

If you have any questions, please call the school at 860-347-2978.
